New Patient Registration

To register at Cedar House Surgery or our branch surgery Dumbelton Medical Centre, you will need to come into surgery and request a registration pack which will consist of the GMS1 form and a patient questionnaire. This will require completing before handing back to a member of the reception team. We will at this point check all the information has been provided so we can successfully register you onto our system. If information has not been provided, we will need to hand the registration pack back to you and ask for you to complete this before we can register you.

Once we have all your information filled out on our documents, we will ask you to use our height, weight and blood pressure machine at Cedar House Surgery or if you are registering at Dumbelton Medical Centre, we will book you in for an appointment with the practice nurse who will see you for a new patient health check.

If you are on any medication please state this when giving the registration pack to the receptionist if they have not already asked you. We ask that before leaving your previous surgery, you ask for a supply of medication to cover you while you are registering at a new practice. Once we know how much medication you have left, we will then pre-book you an appointment to see the GP before your medication will run out for a new patient medical - patient on medication

We will only be registering patients who are not already registered at a practice in the local area.
